The Your Health Matters team proactively visit patients across Keswick and Solway who have been identified as at risk for hospitalisation or frailty. They conduct holistic assessments around what matters to the patients, supporting them to stay well and at home for longer. They do not take referrals as they identify patients based on GP local knowledge and searches within our Clinical system. The team have seen that those patients who are engaged with the service have reduced the number of contacts that they previously had with either their GP or secondary care by 50%.  

They have supported patients with not only their health but also with how to manoeuvre the new NHS digital systems and how to do their shopping online.
