We have a number of PCN additional teams that have been recruited to support the traditional general practice model, these are;
Frailty Team – made up of ACPs, Specialist Frailty Nurse’s and Care Coordinators who provide acute and routine visits to our elderly patient population to ensure that they have routine health checks, care plans completed and vaccinations up to date.
This team is also now the lead for our Primary Care Network / Practices with the Enhanced Health in Care Homes work – the ACPs complete weekly ward rounds with the support of the Lead GPs and the Care Coordinators ensure that all residents have up to date reviews and care plans in place. They also lead on the weekly PCN Multi-Disciplinary Team meetings.
Clinical Pharmacists Team – we have a team of Clinical Pharmacists and Pharmacy Technicians who support the Practices and staff with medicine related issues and queries. They undertake medication reviews, manage patients with complex polypharmacy needs, care home patients medication reviews and people with multiple co-morbidities.
First Contact Practitioner (FCP) Musculoskeletal (MSK) – We have a team for FCP MSK staff who work based in our 5 practices and deal as a first contact with patients with MSK complaints, they support the patient to ensure we access appropriate diagnosis and treatment plans ensuring better self-management of their conditions.
Social Wellbeing Link Workers – this team support patients with non-medical issues using a holistic approach to improve people’s health and wellbeing.
Children and Young people Social Prescribing Team – employed via Barnardo’s to support school aged children who are need wellbeing support, this is part of the Link service
Mental Health Worker – working in practices to deal with lower level first contact for mental health issues.
Memory Support Worker – Care Coordinator working within Practices to support patients with mild memory problems and ensure reviews and appropriate referrals to the Memory Team are made.
